Chemical Dependency Evaluation: Cost $40.
A comprehensive history taken during an individual face to face session with a Chemical Dependency Counselor. The history will include all major life areas and result in a DSM IV TR diagnosis and recommendation sent to the referral source within 24 hours.



Intake:
Intake & enrollment is required as a first session for all group and individual programs excluding Basic DWI Intervention. During Intake and enrollment the client and staff member will complete all facility and state required forms, including but not limited to client rights, consent forms and grievance procedures.



Re-Intake :
Client's who wish to re-start a program will be charged an additional fee if they are re-enrolled due to discharge for failure to complete (including but not limited to non-compliance or absences).



Group Sessions:
Drug and Alcohol Awareness:

A 10 session education program to address aspects of chemical use, abuse and addiction. Topics will include drugs of abuse, options for healthier decision making and lifestyle changes. This is only an education program and should not be viewed as treatment; therefore, a client does not have to meet the criteria for a substance use disorder to benefit from this class.



Drug and Alcohol Education:
A 12 session education program to address chemical use, abuse and addiction. Topics will become more in depth than the awareness program to include discussion of options for recovery programs and support system building. This is only an education program and should not be viewed as treatment; therefore, a client does not have to meet criteria for a substance use disorder to benefit from this class.



Stress and Anger Management:
A 12 session minimum education program that focuses on causes for anger, solutions to events that induce and increase stress. Anger and stress resolution tools will be evaluated through client group participation, facilitator assessment, and workbook/homework assignments. This program will be extended through referral consultation and/or client minimum progress.



Life Skills:
A 4 session program to provide education regarding social skills, coping skills, boundaries and better decision making.



Relapse Prevention Counseling:
This 8 session program is appropriate for the client who has had treatment or counseling in the past and might have had a recent relapse or high risk relapse behaviors. A relapse prevention workbook will be covered as well as a plan of action for relapse prevention. Random urine screening is a requirement in this program.



Continuing Care Program:
An 8 session program (or length that is recommended by the facility just completed) that is appropriate for a client who has recently completed an inpatient or outpatient treatment program requiring additional support. Random urine screening is a requirement in this program.



Outpatient Counseling Program:
A 12 session group counseling program to include information related to chemical abuse and dependence. Sessions will focus on disease education, support system implementation, and identification of relapse risk. Random urine screening is a requirement in this program.



Supportive Outpatient Counseling Program (SOP):
This is a 18 group session and monthly individual session minimum program. The client will attend group a minimum of once weekly and individual a minimum of once monthly. Client will attend group no less than two times each week. This program will focus on disease education, relapse prevention planning and support system building. The client will be required to attend 12-step meetings, obtain a sponsor, complete a workbook, and develop a relapse prevention plan. Random urine screening, case management and one individual session each month is required in this program. This program will be extended/augmented in the event of a positive urine screen, reported relpase or failure to progress.



Intensive Outpatient Treatment Program (IOP):
A 32 group session and monthly individual session minimum program. The client will attend four times each week for the first phase (first four weeks), two times each week for the second phase (second four weeks), and one time each week for the third phase (last eight weeks). Client services will be provided at a minimum of nine hours each week. This program will include three phases (primary, secondary and continuing care). The primary phase of the program will focus on disease identification and education, support system planning, family system pattern identification and values clarification. The secondary phase will focus on support system implementation, family and/or significant other involvement, and relapse prevention skill building. The final phase will consist of a continuing care support program that continues to monitor and develop healthy decision making skills. The client will be required to attend 12-step meetings, obtain a sponsor, complete a workbook and a relapse prevention plan. Random urine screening, case management and two individual sessions each month are required in this program. This program will be extended and/or augmented in the event of positive urine screen, reported relapse or failure to progress.



Individual Sessions:
Individual sessions are required for treatment programs and offered to active clients at a discounted rate. These sessions will focus on needs specific to the individual client and their treatment planning. In the event of positive urine screens additional individual sessions may be required. Current active clients may request individual sessions with a counselor at anytime during their program in addition to any required sessions. A client may also enroll in individual sessions without being involved in programs. The individual sessions are provided by licensed counselors and/or counselor interns in 50 minute intervals by appointment. The specialization of the counselor and the need of the client will be considered.



Anti-Theft/Theft Prevention:
A 6 session program focusing on life skills and healthy decision making. This class will be offered at all locations and client can attend an existing group to meet requirement. We will offer this in an all day 6 hour format soon.



Urine Screens:
Random urine screens are given in many programs. Urine screening can also be conducted at referral request or the request and payment of the client.
